Acalculous Cholecystitis


Abstract
This is a retrospective analysis of a 5-pear experience about 72 cases of acalculous cholecystitis who were treated surgically at the Chonnnam National university hospital during the 5 years period from Jan. 1977 to Dec. 1982, the following results were obtained.
1) Among 530 patients who needed the biliary operation, acalculous cholecystitis was found in 72 cases(13.5% ).
2) The male to female ratio was 1.18 : 1. The age range was from 4 to 82, and the prevalent age was 6th decade(27.8%).
3) Histologically, acute cholecystis was 31 cases(43%) and chronic cholecystitis 41 cases (57%). Acute suppurative cholecystitis was 15 cases(20.8%) and gangrene with perforation 7 cases(9.7%).
4) Possible etiologic factor were: a) primary infection 22 cases(30.6%), b) parasite infestation 9 cases(12.5%), c) extrabiliary operation 5 cases(6.9%), d) congenital anomaly 4 cases(5.6%), e) inflammation of adjacent organs(2.8%), f) unknown cases(41.7%).
5) There was no difference in symptoms and signs between the calculous and acalculous cholecystitis.
6) The bile was cultured in 35 cases, the most common organism was E. Coli and the next was Salmonella and Klebsiella.
7) Operative motality was 4.2%.
